If your Driving Licence or Learner Permit has been lost or stolen, you can apply for a replacement.
Replace a lost or stolen licence/permit that was still in date
To get a replacement, follow these steps:
Step 1: Complete the Application Form D800
Application Form D800 is available from public libraries and Garda Stations.
Alternatively contact us at the address below and we will send you form D800.
This form must also be stamped at a Garda Station.
Step 2: Return the form to us
Send the application form back to us at the address below.
Please also enclose the following:
- Proof of your identity. This could include your:
- Original Irish or EU passport.
- Original EU national identity card.
- Original Garda National Immigration Card.
- Two identical passport type photos, signed by you on the reverse.
- A cheque or postal order for €10, made payable to ‘Dublin City Council’. Do not enclose cash.
Replace a lost or stolen Driving Licence that was out of date (expired)
To get a replacement, follow these steps:
Step 1: Complete the Renewal Application Form D401
Renewal Application Form D401 is available from public libraries and Garda Stations.
Alternatively contact us at the address below and we will send you form D401.
This form must be stamped at a Garda Station.
Step 2: Complete the Lost or Stolen Expired Licence Form D8A
This form must be stamped at a Garda Station.
Step 3: Return the form to us
Send the application form back to us at the address below.
Please also enclose the following:
- Two identical passport type photos, signed by you on the reverse.
- A cheque or postal order for the appropriate fee made payable to ‘Dublin City Council’. Do not enclose cash.
- 1 year licence: €15
- 3 year licence: €15
- 10 year licence: €25
Replace a lost or stolen Learner Permit that was out of date (expired)
To get a replacement, follow these steps:
Step 1: Complete the Renewal Application Form D201
Renewal Application Form D201 is available from public libraries and Garda Stations.
Alternatively contact us at the address below and we will send you form D201.
This form must be stamped at a Garda Station.
Step 2: Complete the Lost or Stolen Expired Licence Form D8A
This form must be stamped at a Garda Station.
Step 3: Return the form to us
Send the application form back to us at the address below
Please also enclose the following:
- Two identical passport type photos, signed by you on the reverse.
- A cheque or postal order for €15.00, made payable to ‘Dublin City Council’. Do not enclose cash.
